arc Physio Pilates Terms and Agreements
I understand that
I am consenting to suitable treatment, that has been adequately explained to me by my Therapist.
I will be charged in accordance to the current fee structure (arc Wellness Studios reserves the right to alter the fee structure without notice).
A fee of $70 will be charged if I do not give 24+ hours notice before cancelling an appointment.
Cancellations will not be accepted on social media or text message.
If I arrive late to my appointment, my appointment will be shortened but the full fee will be due.
Information related to my treatment may be provided to the Accident Compensation Corporation (ACC) or relevant health providers if requested.
If we believe there is a serious risk of harm to you or to others, we may need to share relevant information with appropriate professionals (such as your GP, mental health services, or emergency services) to help keep you safe.
I am consenting to further information regarding my treatment or condition being collected from or sent to other related health professionals when required - by email or post.
If ACC decline my claim or to extend my claim I am liable for the total fees.
Clinical administration that needs to be completed outside of the appointment, may incur a fee. (eg telephone calls, written reports, referral letters, ACC32 applications)
I am agreeing to pay my account by the end of each visit. An official receipt will be issued if requested.
Disclaimer / Liability Waiver
I am over the age of 16 OR signing as legal guardian of someone under the age of 16.
I have answered these questions to the best of my knowledge and know of no reason why I should not undertake treatment that may involve a course of exercise. If there are changes to my health, I am responsible for seeking a separate medical opinion before continuing with my treatment programme.
I understand it is my responsibility to inform my Therapist of any changes to my medical condition.
I understand that while arc Wellness Studios takes every care to ensure safe practices, there are inherent risks in any exercise programme. arc Wellness Studios accepts no liability whatsoever for any injury or illness incurred during related sessions and/or by failure to notify of any existing or developing health problems or special requirements.
The undersigned agrees not to hold any arc Wellness Studios employee liable whatsoever for any injuries or illness sustained or in connection with any training sessions or thereafter.
I take full responsibility for my personal belonging.
Pricing
All prices are quoted in New Zealand Dollars ($NZD) and all transactions are processed in New Zealand Dollars ($NZD).
Prices are subject to change without notice. We reserve the right to amend prices and printing errors. Prices may increase from time to time to reflect increases in cost and benefits.
All purchases are final. arc Wellness Studios does not offer refunds, transfers, suspensions or extensions on services, courses or products for change of mind, change of address or any other reason. We will consider extensions on class concessions upon sudden injury or illness with a medical certificate.
All concessions are non-transferable.
arc Wellness reserves the right to bill the client the cost of any additional bank fees which may incur from disputed or failed transactions regarding a clients account.
